It is better to book in advance and get prepared yourself. You will have 3 to 6 people in a group and it is always nice to have small group. Sweet and best! Yes, you will be visiting at Kalapatthar (5500m) which is higher than Everest Base Camp.

how likely is it that the trip from KTM to Lukla and back will be the direct flight as opposed to the 5-6hour drive then flight, in early April 2023

As per Nepal Aviation Authority, all our Lukla flight will be from Ramechhap Airport instead of Kathmandu to avoid Air traffic and flight delayed. However, there may have still few flight going from Kathmandu at the early in the morning. Therefore we will look for those flights if possible if not we have to fly from Ramechhap which is about 3/4 hours drive from Kathmandu. For travelers, i would suggest to go to Ramechhap a day before their flight to Lukla. So that they can explore around Ramechhap as well.

  Maybe it would be better for aclimatization to do Bhutan and/or Tibet before doing the Everest! I've heard from a friend that he hiked unto just above 6000 m. That is not possible, is it?

  No, Nov is good time with Clear Sky! Snow will be expected from End Dec to middle April. But you need to have prepared with snowfall as well. Due to Global warming things have been changed!
