This is a small Group tour. I will definitely arrange the transportation back and forth from Quito to Tena. This will be shared transportation that picks you up from your accommodation and will drop you off at the Lodge, this will have an extra cost and departs from Quito and Tena at 5 am, 12:00 pm, and 5:00 pm, you can choose, but I would recommend early in the morning this way you still get to the lodge early and can enjoy it to the fullest.

How active is this, looking for something easy for an 82-year-old

The lodge at Tena has no heavy activities, is more of a relaxing trip. I can recommend them to people of that age. The transfer from Quito to Tena will be 3:30 hours (Private transfer), private transport for the lodge will pick you up from the hotel to Tena, which costs $288 (Roundtrip). I do not recommend going on a public bus, sometimes could be a little tired.

About your question How do you get to it from Quito, I would like to clarify that there are no flights available from Quito to Lago Agrio, what's available and guaranteed is an overnight shuttle bus that leaves Quito the night before your Amazon trip. It departs from Quito around 11 PM and it additional costs $60 per person roundtrip.
