I am open to see the options from Milan, maybe Florence and Rome or another possible route. This can be guided by bus or train. 3 adults +18, 3 or 4* hotels, nothing luxury

After the Fantasia Lakes tour departing on October 12th, you could add one of our Fantasia Classic 8-day tours, which includes Florence, Venice again, and then Rome. The tour would be a regular group with a tour leader, rather than a small group. The only point to consider is that the tour would start from Rome very early on October 19th. Therefore, I would recommend returning to Rome on October 18th, instead of spending the final night of the Lakes tour in Milan, so that you can join the group on time. Alternatively, the next available departure is October 24th.
