I was wondering if there are other people booked on it for the 23rd? Also, I was looking at flights & I wouldn't get in until 10pm so if I arrived a day early & stayed at an airport hotel that night, could I be picked up the following morning instead? That way I would have a full day of exploring Tirana then.

Is it possible to end my tour in Sarande and stay an additional night in the accommodation at Sarande instead of Tirana?

Yes you can end the tour in Saranda. It will be a 20 EUR supplement for the accommodation in Saranda on the last day.

Is September 18 considered Day 7 of the tour, and are airport transfers to Tirana included on that day? What is the check-out time from the hotel on September 18? I have a later flight that day and want to plan accordingly. I'm scheduled to arrive in Tirana around noon on September 11—will I miss the walking tour planned for that day?

Tirana transfer is included in the program services. Check can be anytime on day 18th. The tour starts on 12 sept not on 11th. If you want to have pre-start tour day, it is ok, but you need to add one more extra night for the 11th of Sept.
